Transition Discoveries

In Pennsylvania, we have partnered with the Transition Discoveries team to help local education agencies (LEAs) implement systems level change for secondary transition programming. The goal of the Transition Discoveries project in PA is to improve transition practices, programs and services at a systems-level across schools, agencies and community partners through community action planning & implementation between young people, families and transition stakeholders.
