Key Components of Transition Planning
Transition Planning must include each of the following components.
Begin at Age 14
In Pennsylvania, Transition Planning must begin by age 14, or younger if determined by the IEP team.
Be Individualized
The transition planning process looks different for every student. It must be individualized based on the strengths, preferences, and interests of the student. Goals should be considered for postsecondary education and training, employment, and independent living.
Be Collaborative
The transition planning process should include the student with a disability, the student’s family member(s), educators, and service providers.
Planning For The Future
The Planning for the Future Checklist is a great place to begin. The checklist is designed to support students with disabilities, as well as their families and IEP team members, to ensure they are reviewing all aspects of transition planning at age appropriate intervals.
